If you are adding a mobile clinic without a street address, please include the city, state, and ZIP for its service area. Please note that mobile clinics without a street address will only appear on the List View of the Find a Therapist page and not on the Map View.

Adding email address here is optional and it will only be used if we have questions regarding the clinic listing you have submitted. Your email address will NOT appear in our listings or be sold to third-parties.

CRI Graduates

Click the "Add graduate" button below to add all CRI Graduates affiliated with this clinic via this form. There is a limit of 10 graduates you can add to one listing. Please be sure to include all credentials for each graduate in the certifications field.
